Finalize version 2 protocol

- There have been several requests for Matlab OpenIGTLink interface.
- Prototype Matlab/Octave interface is available.

- 10:00am-11:30am: Discussion: define protocol version 2.
- 2:00pm-3:00pm: Tools, libraries, etc.
